Post Project Survey & Project Release Form

Thank you for entrusting me with the project we just finished. I'm always trying to improve myself and my business. It would certainly help me out if you take time to jot down some of your thoughts. Both positive and negative comments will help me improve. I know life gets busy. If I don't get a submission within 2 weeks of receiving payment, I'll assume that the project can be used as I would like as a portfolio piece on the website, etc. Thank you for your time!
